We performed a retrospective cohort study to evaluate the incidence and associated risk of renal failure in patients receiving furosemide in the acute phase, acute phase of nephrotic syndrome, or acute renal failure. This study was approved by our Institutional Review Board.
Renal function, acute nephrotic syndrome, and acute renal failure were identified in 2200 patients who received furosemide. The creatinine clearance and serum creatinine levels were determined in 5200 patients, of which 10% were administered furosemide. All patients with acute nephrotic syndrome were followed for a median of 7.5 years. No significant differences in creatinine clearance between the groups were found. The incidence of renal failure in furosemide users was 9.6%. The estimated glomerular filtration rate was 4.3 mL/minute/1.73 m2 and the estimated glomerular filtration rate was 6.9 mL/minute/1.73 m2 in the furosemide group and the placebo group, respectively. The estimated glomerular filtration rate decreased with furosemide use. We conclude that the use of furosemide in patients with nephrotic syndrome and acute renal failure is associated with an increased risk of renal failure. The risk is probably higher in patients on furosemide who are receiving furosemide compared with those receiving placebo. There was no significant association between renal failure and furosemide exposure in the acute phase.
We also performed a retrospective cohort study to evaluate the association of renal function and acute nephrotic syndrome. Patients who received furosemide had an incidence of a creatinine clearance <0.01 mL/min/1.73 m2, or serum creatinine level <0.01 mL/min/1.73 m2, and serum potassium level <1.5 mmol/L.
The data from this study were used for sensitivity analyses. All the available data were available from the literature. The following risk factors for renal failure were considered in the analyses: age ≥50 years, male gender, and concomitant use of potassium-sparing diuretics, renal impairment, and concomitant use of diuretics, and serum creatinine level <1.5 mmol/L. There was no statistically significant difference in the incidence of acute renal failure between furosemide users and placebo users. The incidence of acute renal failure was 9.6% in the furosemide group and 5.7% in the placebo group, and was higher in the furosemide users than in the placebo users.
We also evaluated the association of renal function with acute renal failure.
